Substernal goiter is an enlarged thyroid gland, harboring a component extending into the mediastinum. Surgical management requires genuine and rigorous preoperative planning as physicians could encounter prospect of gland coming close quarters with vital intrathoracic structures. The neck chest multiplanar cross-sectional imaging provide essentialness extracervical approach for procedure. In present study, 57-year-old female who admitted intermittent dyspnea dysphagia huge reported. labs were reported within normal limits video laryngoscopy displayed no pathologic finding. Her sonography revealed multiple nodules without determining most proximal border left lobe. computed tomography depicted substernal lobe, till innominate vein sutureless total thyroidectomy by collar incision median sternotomy was performed. We would recommend just considering to divide meticulously superior arteries veins separately exploring fibrous Ligament Berry, that is, true its safe relationship recurrent laryngeal nerve in Thyroidology.
